GWEP-CC Age-Friendly Case Study Series
GWEP-CC Age-Friendly Case Study Series: The Geriatrics Workforce Enhancement Program Coordinating Center’s (GWEP-CC) Age-Friendly Case Study Series features a collection of cases accompanied by podcast episodes developed by Geriatrics Workforce Enhancement Programs (GWEPs) and their partners. The series explores three themes in Age-Friendly Health Systems: integrating the 4Ms with electronic health records (EHRs)/electronic medical records (EMRs), tribal, advancing care for tribal organizations and underserved and/or rural (TTOUR) populations, and implementing Age-Friendly practices into nursing home care. Case study authors are from GWEPs who were funded by Health Resources and Services Administration (HRSA) in 2024. Age-Friendly Health Systems is an initiative of The John A. Hartford Foundation, the Institute for Healthcare Improvement (IHI), and the American Hospital Association.
